Constructed of flat and rusticated limestone and sheet metal in the Renaissance Revival style. The building features a deep dentiled cornice, pedimented entry bays with small single-story entry porticos supported by small double Corinthian columns and a limestone balustrade, square Corinthian pilasters at building corners, and an ornamental clocktower with cornice, brackets and crowned by a lantern and Lady Justice statue.

The Milam County Courthouse in Cameron was designed in the Renaissance Revival style by Austin architect A. O. Watson of the firm Larmour and Watson and built of local stone between 1890 and 1892 by contractors Lee and Plummer. Its cornerstone was dedicated on July 4, 1891, and the building was occupied in April 1892; a clock tower topped by a statue of Justice was added in 1895, removed in 1938, and reconstructed during an early-2000s restoration. The courthouse and the nearby 1895 Milam County Jail were together added to the National Register of Historic Places on December 20, 1977.
